You must have an account application form filled out and signed for memory.com to accept purchase orders and company checks as payment methods.
You have two ways to obtain a copy of the memory.com account application form.

  • View and print the form online then fax it to us at 956-971-8595. (Adobe Acrobat required)
  • Email the form to: Credit Application and have the application emailed or faxed to you within 1-2 hours. (Please include fax and email address in your response)
